Primary Health Center is the Full form of PHC. Community Health Centers is a full form of CHC. 

Features and Functions of PHC and CHC:

• In Developing Countries Like India or Afghanistan PHC or primary health center is affordable with the basic structural and functional unit of public health services. In 1978 the World Health Organisation outlined in the Alma Ata Declaration that Primary Health Center is the center that will provide affordable, inexpensive, and most basic health services to the citizens of the country in Primary Health Centers. 

• In simple words, primary health centers are state-owned rural or urban health facilities in India. Primary Health Centers are generally basic small physician clinics that have facilities and infrastructure for minor surgeries. 

• Most Community Health Centers or CHCs are collections of an interdisciplinary group of healthcare providers who use electronic health records. Community Health Centers have been in Ontario for more than 40 Years.

•  The Community Health Centers in the world was established in Canada at Mount Carmel Clinic in 1926. Community Health Centers are basically non-profit organisation and private organisation which offer health facilities depending on the impact and involvement of consumers.

•  Community Health Centers offer fundamental health facilities, especially in those areas where there are no or few availability of primary health centers in any region.
