Myxedematous cretinism

Myxedema is another word for hypothyroidism that has progressed to a severe degree. The problem arises when your body's ability to generate thyroid hormone is reduced to an unacceptable level.

The thyroid is a tiny gland that is located just in front of your thyroid gland. It causes the release of hormones, which assist your body in regulating energy and controlling a wide range of functions. Myxedema is a complication of undetected or untreated severe hypothyroidism that causes swelling. The term “myxedema” can refer to hypothyroidism that has progressed to a severe degree. However, the term is also used to describe the skin abnormalities that occur in those who have severe hypothyroidism at an advanced stage.

The most common skin modifications are as follows:

  1. Swelling of your face, which can include your lips, eyelids, and tongue, as well as swelling and thickening of skin anywhere on your body, but particularly in your lower legs
  2. A myxedema crisis, which is a medical emergency in the case of severe hypothyroidism, can occur in the case of advanced hypothyroidism. This life-threatening condition is referred to as “myxedema coma” in some circles; however, this name has been superseded with “myxedema crisis” because the presence of a comatose state is no longer required to diagnose the condition in question.

What exactly are the signs and symptoms of myxedema?

Other indications and symptoms of severe hypothyroidism, in addition to skin abnormalities, can include the following:

  1. blood pressure that is low in the systolic and high in the diastolic
  2. heart rate that is too low
  3. cold intolerance in those with thin or brittle hair
  4. goitre is a term used to describe someone who has a goitre (enlargement of the thyroid gland)
  5. low levels of energy and lethargy
  6. gaining weight is a weakness
  7. intolerance to the cold
  8. constipation
  9. depressed state of mind

What are the signs and symptoms of a myxedema attack?

Myxedema crisis happens when your body’s ability to endure the alterations caused by severe hypothyroidism is exhausted, and as a result, it adapts by decomposing. An emergency medical response is required because this is a life-threatening situation. Symptoms of myxedema crises can appear alongside the signs and symptoms of severe hypothyroidism, and might include the following:

  1. lower levels of oxygen in the blood (respiratory depression)
  2. sodium levels in the blood are lower than normal.
  3. thermonuclear hypothermia (low body temperature)
  4. shock that causes disorientation or mental sluggishness
  5. low quantities of oxygen in the blood
  6. high levels of carbon dioxide in the blood
  7. coma\seizures

What is the cause of myxedema?

Hypothyroidism is a condition in which the thyroid does not operate properly. This could be caused by one or more of the following:

  1. surgery to remove your thyroid thyroidectomy radiation therapy for cancer certain medications, such as lithium or amiodarone (Pacerone) an autoimmune condition, such as Hashimoto’s thyroiditis iodine deficiency or excess of iodine during pregnancy immune system medications, such as those used in cancer treatment
  2. Undiagnosed or untreated severe hypothyroidism results in myxedema, which can be life-threatening. It can also develop if a person stops taking their thyroid medication for an extended period of time. It affects more older people and women than men.
  3. Myxedema is a skin disorder caused by the accumulation of chains of sugar molecules in the skin. These chemicals are attracted to water, which causes them to swell. The hypothyroidism that is causing these skin changes is a byproduct of the condition.
  4. Having a lengthy history of hypothyroidism is a risk factor for developing myxedema crises. It occurs more frequently during the frigid winter months. It can be triggered by any of the following: severe sickness, such as a heart attack or stroke infection trauma some medications that depress the central nervous system exposure to cold stress halting hypothyroid treatment medication

What is the procedure for diagnosing myxedema?

Your doctor will suspect that you have severe hypothyroidism based on your symptoms. Your doctor can confirm this with the use of blood testing.

The amount of thyroid-stimulating hormone (TSH) produced by your pituitary gland is measured by a thyroid-stimulating hormone (TSH) test. If your thyroid isn’t producing enough thyroid stimulating hormone (TSH), your pituitary gland will enhance TSH production. In other words, having a high TSH level could indicate that you have hypothyroidism.

A thyroid stimulating hormone (TSH) test is usually performed in conjunction with a thyroxine (T4) test. Test for thyroid hormone T4, which is produced directly by the thyroid, is performed using this method. The condition known as hypothyroidism is characterised by low levels of T4 along with a high level of TSH.

Myxedema crisis is a medical emergency that requires immediate attention. TSH and T4 levels should be evaluated as soon as hypothyroidism is suspected. Treatment can begin as soon as feasible after the diagnosis. The physical examination is frequently used to make the initial diagnosis.

Other signs and symptoms of severe hypothyroidism will be observed by emergency medical personnel, including:

  1. skin that is dry and hair that is sparse
  2. Hypothermia can cause edema, particularly in the face and legs.
  3. A goitre is a surgical scar that may result from a thyroidectomy.
  4. Low blood pressure and a fast heart rate might lead to confusion.
  5. lower levels of oxygen in the blood

If your doctor feels that you are experiencing a myxedema crisis, you will begin receiving thyroid hormone replacement medication. An intravenous line is the preferred method of accessing the bloodstream (IV).

Other blood tests will be ordered by your doctor in order to provide a complete picture of your body’s systems. It is likely that you will also require a CT scan of your brain.

During this process, your critical functions and degree of consciousness are also continuously monitored on a continuous basis. You’ll need to be admitted to the hospital and treated in the intensive care unit (ICU) until your condition stabilises.

What are the risks associated with myxedema and its complications?

Thyroid hormone is required for the proper functioning of cells. As a result, severe cases of hypothyroidism can cause your metabolism to slow down and your body’s ability to utilise oxygen. A negative impact on practically all biological processes and body systems is possible as a result of this. As a result, myxedema can cause the following symptoms:

The effects of hypothermia include edema and fluid buildup.

Drug metabolism is impaired, resulting in overdose on drugs.

Pregnancy complications, such as miscarriage, hypertension, stillbirth, and birth abnormalities, are discussed.

failure of the heart

Renal difficulties are a common occurrence.

depression

coma\death

Treatment for myxedema

The medication levothyroxine is used to treat hypothyroidism, which is characterised by low levels of the thyroid hormone T4 (Levothroid, Levoxyl). Symptoms become more manageable once T4 hormone levels are restored, though it may take many weeks for this to happen. The likelihood is that you will be need to take this medication for the rest of your life.

Conclusion

Medical professionals consider myxedema crises to be a medical emergency that demands quick intervention. Those who are experiencing a myxedema crisis should be treated in an intensive care unit. They are continuously checked for their heart rate and respiration. Other drugs, such as steroid therapy and other pharmaceuticals, may be required in addition to thyroid hormone replacement.
